Centers Choice is a private pay home care company that is seeking passionate RN's for in-home care management for complex chronically ill clients. As a Home Care Nurses you will assist in managing direct care to our clients and their families per the established plan of care. We are growing rapidly and have immediate per-diem opportunities for both skilled visits such as wound care, IV therapy, and/or shift work that ranges from 3-12 hours day or nights.

• Valid New York State RN License
• 2 References
• CPR Card
• Professional Insurance
• Minimum one year experience
• Physical - (can be performed remotely)
• PPD/QuantiFERON within the past 6 months
• Proof of MMR Immunization
• 2 References
