GENERAL STATEMENT OF DUTIES

Assists in the delivery of rehabilitation services under the supervision and direction of rehabilitation personnel within the disciplines of Physical Therapy, Occupational Therapy and Speech-Language Pathology as allowed by state and federal regulations. Transports residents on active therapy program, as well as assists in therapy related activities assigned by rehab personnel. Responsible for clerical duties assigned by the Rehabilitation Director and other rehabilitation staff for timely and organized maintenance of medical records, soft charts and other documentation. Basic computer work, data entry and general housekeeping duties to maintain the organization and cleanliness of the rehabilitation department are additional essential functions of this position.

SUPERVISION RECEIVED

Direct supervision from the Rehabilitation Director, Regional Director of Rehabilitation and when appropriate, other staff therapists including Physical, Occupational and Speech Therapy.

JOB FUNCTIONS

PRINCIPAL DUTIES AND RESPONSIBILITES include the following:

  1. Assists in direct therapy service provision by carrying out delegated, selected patient related tasks under the direct supervision of a therapist allowed by state and federal law.
  2. Transports patients/residents in compliance with patient/residents individualized care plan/kardex to and from therapy department safely and efficiently. Transportation responsibilities include: advancing to the next resident if previous resident is not ready (notifying therapist when returning to department), ensuring that residents feet are on the foot rests when applicable, ensuring resident is in correct wheelchair/alternate positioning equipment in accordance with patient/residents care plan or by checking name tag on back of wheelchair when present, ensuring appropriate seat belt or seat alarm is in place, ensuring oxygen tanks have sufficient oxygen and notify nursing and /or therapy when low, transports one resident at a time, and upon completion of therapy session returns resident to unit, nursing station or dining room, if requested. Do not return resident to their rooms unattended unless instructed.
  3. Escorts independently ambulatory residents to the rehabilitation department when requested.
  4. Provides additional physical help for patient transfer and safety needs under the direct supervision of a therapist or therapist assistant. Rehabilitation Aides do not perform clinical tasks reserved for licensed therapists or therapist assistants.
  5. As required, will assist residents in self-care activities such as: toileting, bed mobility, transfers, dressing, ambulation, personal hygiene and feeding following established care plans for the resident.
  6. Prepares hot packs and cold packs for application by therapy staff per facility protocols. Does not apply hot packs or cold packs to residents, only assist in the preparation of the application.
  7. Maintains cleanliness and safety of the rehabilitation departments physical environment and assigned equipment.
  8. Oversees maintenance of equipment in accordance with infection control safety guidelines.
  9. Responsible for maintenance and replenishment of daily needs of clean linen and other commonly used items. Returns used/soiled linens to housekeeping per facility protocol.
  10. Completes assigned clerical duties, such as computer data entry, photocopying, filing, answering the phone, retrieval of therapy related documentation and other paperwork.
  11. Maintains a record of equipment maintenance and cleaning schedule as assigned by the therapist manager/s.
  12. Performs routine inventory and informs the Rehabilitation Director/s of stock replenishment needs.
  13. Performs other duties as assigned by his/her supervisor.
  14. Maintains current temperature logs for hydrocollator, refrigerator, freezer and paraffin bath. Reports any irregularities to the Rehabilitation Director immediately. Retains temperature logs for historical review.
  15. Participates in quality improvement activities as assigned by the Rehabilitation Director and/or Regional Director of Rehabilitation.
  16. Responsible for service and operational excellence of all assigned activities to ensure the delivery of quality services required to meet or exceed the expectations of patients / residents, families, caregivers and customers.
  17. Exhibits courtesy, compassion and respect to patients, residents, families, caregivers, visitors, physicians, administrators and co-workers.
  18. Adheres to established confidentiality standards, including but not limited to HIPPA regulations and internal company policies.
  19. Maintains universal precautions.
  20. Regular and prompt attendance.
  21. Follows dress code: no jeans, no shorts, no tank tops, scrubs are acceptable, clean sneakers are acceptable.

MINIMUM POSITION QUALIFICATIONS

  1. High school diploma or equivalent. One year experience in a health care setting preferred. Current certification as a Certified Nurse Aide (CNA) is required.
  2. Writes and understands English.
  3. Basic computer skills -ability to use Microsoft Office programs such as Word and Excel, use email/calendar function of Microsoft outlook, ability to navigate rehab documentation following training by rehabilitation staff.
  4. Demonstrates effective oral and written communication skills in English.
  5. Ability to function effectively in multi-faceted systems and to interact effectively with all levels of staff and customers.
  6. Demonstrates good organizational and interpersonal skills.
  7. Demonstrates ability to perform basic math skills (i.e. add, subtract, multiply and divide).
  8. Demonstrates ability to perform basic mechanical skills (i.e. use of screwdriver, wrench, pliers).

PHYSICAL REQUIREMENTS

Physical requirements for this position may require light to moderate physical exertion. While performing the duties of this job, the employee is required to stand, walk, sit, stoop, kneel, bend or crouch; use hands to manipulate tools, equipment or controls; reach with hands or arms, balance, lift and perform transfer assists with residents. The employee is required to have visual and hearing acuity sufficient enough to assess resident safety and ability.
